The Change Request Form is an essential tool within any organisation and is used to avoid any unexpected negative impact from a change.
The purpose of the change request form is to manage a change and is used for requesting, approving, and documenting changes within an organisation or a project.
The change request form is typically completed by the individual requesting the change; however it could be completed by another person as a record/assessment of a potential change.
Before a change can be signed off, relevant stakeholders will analyse all the data to determine the impact and potential risks associated with the change. It needs to be clearly defined who can approve a change, ones it has been assessed.
Changes are unavoidable and must be managed to avoid unexpected results.
